The 8 characters before the slash determines you callsign/radio ID.

'K7VE    /John' and 'K7VE    /91AD' as far as D-STAR is concerned are the
same thing (radio/callsign/etc.)

On my radios, I have the following setup:

'K7VE    /John' is the 2820H
'K7VE   P/John' is the 91AD
