 By "forms that are sent" do you mean "anything in my sent folder" ?

 If so, please try deleting your entire message store which will nuke the
 metadata which may be corrupted.  To do that on Windows, go to Start->Run
 and type "%APPDATA%\D-RATS".  In the folder that opens, delete the
 directory "messages".  After that, please try to re-create the problem.
 If you still see it, please capture a fresh debug log and attach the
 entire thing here.
