Comment:

 You should be able to replicate a fair bit of this form in D-RATS.  The
 printable version won't look like it without a stylesheet of course, but
 for data-entry it should work.  You can get to the form builder in the
 File menu and poke around with that.  If you build a form and would like
 it included in the main D-RATS distribution, I'll be glad to do that.

 Since this isn't a bug, I'm going to close this item as such and ask that
 any further discussion go to the mailing list.
