Hi Rob,
you should have gotten forms the first time you ran the program. d-rats makes a directory called .d-rats in your home directory (folder), and makes subdirectories under that. Here's what mine looks like:

Form_Templates d-rats.config maps repeater.config static_locations
Saved_Forms logs messages repeater.log
481 mnementh> ls Form_Templates/
ICS213_OR_WashCoARES.xml ICS213_US_OS.xsl hics260.xml radiogram.xml

I'm in /Users/fligor for the first listing (ls):
If you're not used to using Terminal to access files you might not know how to see these, since the folder starts with a "." it is hidden from your view in the Finder.
you should be able to put the form files into your Form_Templates folder and then use them, if yours is for some reason empty, but you'll need to use the "cp" or "mv" command in Terminal to do so unless you want to change the finder to show all the unix files all the time (if you do, check out http://www.macgeekery.com/tips/view_hidden_files_and_folders_in_finder )
