 Thanks Dan,
  The goal for my form is to just track patient status and transportation.
 The 213 is a great form, but in a MCI it takes too long to fill out in the
 field and will take longer to send as well.

   We are trying to prepare for an MCI exercise in October with over 110
 patients and 4 treatment areas.  So, getting quick patient status to the
 IC and transportation coordinator is key.

   In the Triage area, no more than 30 seconds to fill out the form, about
 12 fields are needed for this.

   The status would show on the IC DRATS page to allow them a quick view of
 status, et..
