You can create the form itself by using the D-RATS form builder <br>
(File->Manage form templates). The XSL is hand-edited in a text editor. <br>
The current set of XSLs have a couple of big chunks of duplicate code <br>
that should be factored out into a common file. This would make it a <br>
little easier to add new ones over time.<br>

So, it sounds like you could use the form editor to create a D-RATS form <br>
for something and then post it here. John or I could then work up a <br>
stylesheet for it.<br>

Are there any common forms I'm missing that would be good to start with?<br>
