You can create the form itself by using the D-RATS form builder 
(File->Manage form templates).  The XSL is hand-edited in a text editor. 
  The current set of XSLs have a couple of big chunks of duplicate code 
that should be factored out into a common file.  This would make it a 
little easier to add new ones over time.

So, it sounds like you could use the form editor to create a D-RATS form 
for something and then post it here.  John or I could then work up a 
stylesheet for it.

Are there any common forms I'm missing that would be good to start with?
