Yep.  If you notice, some of the stock forms (like the ICS213,
radiogram, etc) have a nice "printable" version.  This is accessed by
opening a form of that type and clicking the printable button at the
bottom.  Here is a contrived example I just saved:

The printable button translates the form into HTML, which can be
tailored to represent the look and feel of the original paper form.  To
do this, an XSL stylesheet is required.  The stock forms have one, but
any form you create gets a relatively ugly default stylesheet (which, by
the way, really could be better than it is).

If you can send me the form you've created, as well as a PDF or scanned
image of the real form it represents, I can see about spending a little
time on a stylesheet for it that will make it a little more attractive.

Also, if you have a general-purpose form that other people could use, by
all means, send it to me and I'll include it in the distribution.
