Usually caused by one or both of the following:

- the plug of the programming cable is not fully inserted into the
socket. There can be many causes for this but with these UV-5R like
radio models with the recessed speaker/mic socket, the plastic plug
shell can contact the radio case causing interference that won't allow
the plug to be fully inserted. This is especially troublesome with the
GT-3R style cases.

- a compatible device driver is not present or there is a problem with
the USB connection. This is really common with programming cables
manufactured with Prolific type chips. The chip vendor end-of-lifed
their PL2303HX chip (common in most programming cables) over a decade
ago and removed support for it from their driver causing issues like
this. Recently (towards the end of last year) they did the same thing
for their PL2303TA chip. Also when there is no driver installed (as in
the case when the USB end is not plugged in, the computer is powered
off or rebooting), the radio can be forced into transmit mode by the
programming cable. This is why I cringe when I see someone recommend
programming their radio with the antenna removed!

This is not a problem caused by CHIRP and there is nothing that CHIRP
can do to resolve it.
