Joe,
I cut and pasted your channels into my editor and saved it to a csv file.
It imported fine into my UV-5R template. I then loaded a VX-3 image from
the CHIRP website and imported the same file into it. I made the adjustment
to adjust the imported channels up by 1 since they started with 0 and the
image template started with channel 1. I got the first channel and all the
rest were blank.

There is already an open issue, #875, regarding this. You might add a
comment and attach a debug.log while you are at it.
