The UV-5RE cannot store CTCSS and DTCS at the same time. You'll have
to choose which one you want to use. First set the Tone Mode column
and then set the Tone, ToneSql, or DTCS depending on which Tone Mode
you chose. In Chirp, check View -> Hide Unused Fields to make this
more obvious. View -> Smart Tone Modes might be helpful too.
