 I should mention Excel cannot export in this format.  Every record,
 including the header contains the same number of commas.

 The two error messages are not particularly helpful.  The extra comma in
 the header record reports "CSV Format Mismatch".  Leaving off the "Digital
 Code" field entirely reports "list index out of range".  The latter
 message is very hard to diagnose.
